How to use a content library image in a web form (html5)

As of version 8.7.1, this article is obsolete. The concept should still work, but see this article for the new, easier approach to putting an image on a form.
 
 
How to use a content library image in a web form
  • Add the appropriate image to the content library (if it does not already exist)
  • In the Content Library, edit the content library image you wish to display in your web form
  • Right click on the previewed image and select "copy image URL"
  • Use the copied URL to reference the image based on the format displayed below:

Format:

  • Create a Label question in Design 
  • In the 'Label Text' area, paste the URL for the Content Library image
  • Save and test that your image displays in the web form
