How to use a content library image in a web form

  1. Add the appropriate image to the content library (if it does not already exist)
  2. In the Content Library, edit the content library image you wish to display in your web form
  3. Right click on the previewed image and click on Properties
  4. Copy the Address(URL)
  5. Create a Label question in Design with a html image tag
<img src="URL" width="xx" height="xx" alt="Image Name">
  1. Paste the URL from the Content Library image into the html image tag
  2. Save and test that your image displays in the web form

Example
